Providing Support & Care at Every Stage of The Journey

The Children’s Home provides a wide variety of residential, home-based, and community-based programs designed to address the needs of vulnerable children and their families while supporting their long-term progress. Programs and services delivered by the Children’s Home are obtained exclusively through referrals.

Our Programs

The Children’s Home provides day-to-day, on-campus care at several residential facilities in addition to a wide-ranging spectrum of programs and services delivered in homes and communities across New York State.

From prevention and early intervention to crisis-stabilization and long-term recovery, CHWC’s comprehensive Behavioral and Mental Health Services provide a continuum of evidence-based, person-centered care for children, youth, and adults with complex behavioral and/or mental health needs.

Through an array of community-based programs, including intensive, team-based models, CHWC delivers individualized services to promote stability, strengthen families, and improve overall well-being. Services are provided in homes and community settings, whenever possible — ensuring accessibility, reducing barriers to clinical care, and supporting clients across all stages of need.

CHWC’s Behavioral and Mental Health Services include:

  • 29-I Clinic
  • Assertive Community Treatment (ACT)
  • Youth Assertive Community Treatment (Youth ACT)
  • Child & Family Treatment Support Services (CFTSS)
  • Critical Time Transition Program (CTTP)
  • Health Homes Care Management
  • Home Based Crisis Intervention (HBCI)

When living at home isn’t possible, CHWC’s residential services provide safe, supervised living environments for children and youth, ages 5-21. Offered both in community-based settings and on our Main Campus, these services ensure children receive the individualized care they need in nurturing, supportive settings.

CHWC’s community-based residential services include:

  • Community-Based Group Homes
  • Supervised Independent Living

Rather than being placed in large institutions or hospitals, youth stay in home-like residences, under the direct care of highly trained professionals who help youth build skills, achieve success, and remain connected to school, family, and community activities. In addition to campus-based dorms (Sarah, Rozen, and Ahearn), CHWC’s campus-based residential services include:

  • Residential Treatment Center (male and female)
  • QRTP Diagnostic Center
  • QRTP Raise the Age (female)
  • Non-secure detention

CAPTA CARA supports infants affected by prenatal substance exposure — and the families caring for them. Our team works with parents, caregivers, hospitals, and community partners to develop individualized Plans of Safe Care that address the medical, developmental, and supportive needs of newborns, while connecting families with treatment, recovery resources, and the practical supports that help babies and parents stay safely together.

CHWC’s education programs help youth stay actively engaged, motivated, and connected to learning and advancing their academic careers. Through hands-on support, education-based enrichment, and hands-on learning — our Team creates endless learning opportunities for youth to build confidence, strengthen skills, and experience success both in and out of the classroom.

After School Programs provide a safe, supportive space where children and families can grow, connect, and thrive. Through a blend of therapeutic support, skill-building activities, and meaningful engagement, we meet youth where they are — helping them build confidence, strengthen relationships, and succeed at home, in school, and in their communities.

A division of Children’s Home, the Southern Tier Community Center (STCC), located in Endicott, NY, provides Broome County children, adolescents, and their families with a rich community hub of before and after school programs, services, and resources.

STCC programs include:

  • Learning and Enrichment After-school Program Supports (LEAPS)

In a variety of counties across NYS, our Teams work hand-in-hand with parents and caregivers to build stronger, healthier families and communities. Our goal is to help parents and caregivers to create positive routines, strengthen relationships, set realistic goals, and foster safe, nurturing home environments, where their children can thrive.

Social Care Navigation connects individuals and families to the resources they need to address the social factors that affect health and well-being — including housing, food, transportation, employment, and benefits. Our Navigators work one-on-one with clients to identify needs, make warm referrals to community partners, and follow up to make sure connections take hold. The goal is simple: fewer barriers between people and the support that’s already out there.

CHWC’s Youth Mentoring Programs are designed to address truancy and reduce at-risk behaviors by providing intensive, strength-based, and trauma-informed support to children and youth.

  • Youth Empowerment & Support (YES) & YES for Juvenile Justice (YES-JJ)
  • Vocational Program (Supportive Youth Employment)

Our flexible program model is designed to meet the unique needs of each client and community we serve. In counties across NYS, CHWC partners with local Departments of Social Services and community-based organizations to deliver culturally competent, trauma-informed, family-centered, evidence-based care.

Seneca Preventive serves families involved with the juvenile justice system, working to keep youth connected to their families and communities while addressing the underlying needs that contributed to system involvement. Our team provides individualized support, skill-building, advocacy, and coordination with courts, probation, and community partners — with the goal of reducing further system involvement and supporting healthy long-term outcomes.

Wayne Preventive provides intensive in-home support to families referred through the Wayne County Department of Social Services. Our team works alongside parents and caregivers to strengthen family functioning, address concerns that brought families into the child welfare system, and prevent the need for out-of-home placement. Services are family-centered, trauma-informed, and tailored to the specific needs of each household.
