Community Programs
Community Programs are designed to provide comprehensive help and support for children and their families while they remain in their homes. We work collectively with a number of agencies to ensure that youth and families have a long-term system of support and resources in order to cultivate healthy relationships for positive futures.

After School Programs
Child and Family Treatment Support and Services (CFTSS):
A selection of community based services individualized to meet the needs of the youth and caregivers that we serve
Home and community based services (HCBS)
Home and Community Based Services (HCBS) allows Children’s Home to provide planned and crisis respite to youth on Medicaid. Respite is provided in the community, one of our licensed foster homes, or one of our OCFS licensed facilities. These programs serve a variety of targeted population groups, as deemed eligible through Health Home Care Management.
Children’s Home Preventive Services (CHPS):
Provides family focused, child-centered preventive services for families referred by the Broome County Department of Social Services. Works to strengthen families, improve parent-child interaction, promote self-sufficiency and help raise healthy children.
Health Homes:
Provides case management services focused on coordinating care for children. The model utilizes an interdisciplinary team made up of professionals and informal supports that can surround and support the child in accomplishing his or her goals to live a healthy and happy life. Care Managers have the ability to discuss family identified strengths and needs with various providers in an attempt to unify the efforts of each service to push the child forward in a positive direction rather than each provider pulling the child in different directions. The goal of this program is to prevent adverse childhood experiences including emergency department visits, out of home placement, homelessness, negative school interactions, hospitalization, family relationship breakdowns, and similar negative experiences.
Family Support Program (FSP):
Provides clinical, educational, and recreational services to children in Chenango County. Parents are provided individual, family, and group counseling. The program offers both a therapeutic after-school program and intensive family counseling during the school year and a summer camp program.
Home and community-based services (HCBS) :
Provide opportunities for Medicaid beneficiaries to receive services in their own home or community rather than institutions or other isolated settings. These programs serve a variety of targeted populations groups, such as people with intellectual or developmental disabilities, physical disabilities, and/or mental illnesses.
Therapeutic After School Program (TASP):
A highly successful, intensive family-focused program that provides clinical, educational, and recreational services to children and adolescents. TASP assists youth in finding success in school, in the community, and at home. It also helps families establish better relationships with their children.TASP runs during the school year and for a six week summer session.
